Medical Records

All inpatient and outpatient medical records can be requested from our Medical Records Department. Download and sign the Medical Records Release form and fax it back to (210) 358-5936. The cost for a copy of the medical records for personal use is only 35 cents a page.

Hours of operation are Monday-Friday, 8:30 a.m. to 4 p.m. and they can be reached at (210) 358-3532.

NurseLink - (210) 358-3000

NurseLink is a nurse telephone triage service of University Health System and is staffed 24 hours a day. NurseLink is available to patients of Community Medicine Associates (CMA) and any University Health System-based clinic. NurseLink also serves as the after-hours call center for the Community First Health Plan (CFHP) member. Our staff of professionals can answer your questions about health issues, assist with processing appointment requests or advise you about the appropriate place to go for treatment. For example, if you have a medical problem or question and do not know what to do, call NurseLink first. The nurse may be able to assist you over the phone and make an appointment for you or your family at a University Health System-based clinic near you. Or, the nurse may advise you to go to the nearest emergency room. Remember: in an emergency, always dial 911. Please note: Patients who belong to UT Medicine, may call their call center at 358-3009. Patients who are treated at the UT Medicine Diagnostic Pavilion may call 592-0400.

Resources and Community Services

The Social Services Department at University Health System provides patients with a variety of services. If you need assistance with special housing, transportation, financial aid or home health care arrangements, please call (210) 358-2300. Social Services professionals are available to help you Monday through Friday from 8 a.m. to 4:30 p.m. If you need help after hours or on weekends or holidays, ask your nurse to help you.
