What to Bring

Before you leave home, please be prepared to bring:

• Personal identification (examples include your driver's license, military ID or passport);
• Medical insurance information and identification cards and documents;
• Full amount of medical insurance co-payment, deductibles or fees for service as they relate to your insurance coverage and
• An accurate list of your current medications and any over-the-counter vitamins and herbs you may be taking for your doctor and nurse to review.

To ease your admission to University Hospital, please DO NOT bring valuable items such as jewelry or large sums of money. Leave valuables and excess property at home or with a family member.

A registration process will be completed to record your visit to the hospital. You will be asked for information about yourself, your family, where you work and what type(s) of health insurance coverage you have. You will be asked to sign a consent form that gives the hospital and its staff permission to provide you health care and to bill your insurance so the hospital will be paid directly.
