A social security card or number is NOT required to enroll in CareLink.    CareLink is required to report possible identity fraud. 

Income 

To qualify for CareLink your total household monthly income (based on 2008 guidelines) must not exceed the following:

In order to meet our income requirement, you will need to provide proof of ALL family income for the past 30 days. The following are examples of documents that may be used for this purpose:

• Check stubs for last 30 days showing name, date, employer and gross earnings
• Wage verification form signed by employer
• Document verifying sponsor, who must offer his/her source of income, telephone number and address
• Bank statements, investment accounts and any other form of income
• Final divorce decree showing amount of child/alimony support
• Proof of retirement income
• Copies of documents or checks from federal/state agencies
  
• Texas Workforce Commission (TWC) notice of benefits
• Social Security disability award letter

Identification

In order to meet our identification requirement, you will need to provide one form of identification for each household member. Adults must have picture identification. The following are examples of documents that may be used for this purpose:

• Driver’s license
• State identifi cation card
• Student ID
• Resident alien card
• Birth certificate (children only)
• U.S. immigration card
• U.S. naturalization, citizenship or other federal documents
• Passport
• Credit/debit card with photo

Residency 

In order to meet our residency requirement, you will need to provide proof of Bexar County residency dated within the last 30 days. The following are examples of documents that may be used for this purpose:

• Household bill (electric, telephone, water or cable)
• Lease agreement
• Automobile insurance documents
• Medicaid / Medicare / CHIP letter
• Attorney General letter
• Food stamp letter
